India's latest national health survey has surfaced a troubling paradox: even as obesity and overweight rates climb across the country, the share of Indians who are underweight is not shrinking and, in many states, is actually rising. The proportion of men and women with a normal, healthy body mass index (BMI) has fallen sharply, squeezed from both directions by malnutrition on one end and excess weight on the other.

The findings suggest that India is now living through what public health experts call a "double burden" of malnutrition, a phenomenon in which undernutrition and overnutrition exist side by side within the same population, sometimes even within the same household. Urban areas, the survey found, show particularly high rates of both underweight and overweight conditions, a sign that economic growth and city life are reshaping diets and bodies in starkly unequal ways.

A Legacy of Scarcity Meets a New Excess

India's history with hunger is long and brutal. Famines in the late nineteenth and early twentieth centuries killed millions, and colonial-era administrators openly acknowledged that vast portions of the population never had enough to eat. One British official on the Governor-General's Council conceded that half of India's agricultural population did not know "from one year's end to another what it is to have a full meal." As recently as the 1930s, the average Indian's life expectancy was just 32 years, and food availability per person actually declined by nearly a third between 1911 and 1941 under British rule.

Those historical scars have not fully healed. Chronic undernutrition remains embedded in parts of Indian society, particularly among the rural poor and in states with weaker health infrastructure. What the new survey reveals is that this old problem has not gone away even as a new one, obesity, has arrived with force. Instead of one replacing the other in a clean transition, as public health models often predict, both are expanding their footprint simultaneously.

Cities as the Epicenter of Contradiction

The survey's finding that urban areas show elevated rates of both underweight and overweight is perhaps its most revealing detail. India's cities have long been engines of upward mobility, drawing migrants from the countryside with the promise of better wages and better lives. But they have also become laboratories of inequality, where gleaming apartment towers rise beside informal settlements lacking reliable water, sanitation, or affordable fresh food.

In wealthier urban households, sedentary jobs, increased consumption of processed and calorie-dense foods, and reduced physical activity are pushing BMI upward. Meanwhile, in the same cities, low-income residents, often recent migrants or workers in the informal economy, remain undernourished, unable to afford adequate diets despite living in proximity to relative prosperity. The result is a nutritional split screen: obesity and hunger coexisting within walking distance of one another.

The share of Indians with a normal, healthy BMI has shrunk considerably among both men and women, squeezed between rising obesity and persistent underweight.

What the Numbers Miss

This is not merely an Indian phenomenon. Global development literature has long tracked a widening gap between rich and poor nations in diet and health outcomes. The Brandt Report of 1980, an influential study of global inequality, noted that developing nations, home to the majority of the world's population, consumed a small fraction of global food resources compared with wealthy industrialized countries. That gap in consumption and nutrition between the world's north and south has, in various forms, replicated itself within India's own borders, between its states, and between its rich and poor.

India's economic liberalization since the 1990s has lifted incomes and reduced extreme poverty for many, but it has not distributed the benefits of that growth evenly. The same unevenness that produces billionaires and a growing middle class also leaves large populations without consistent access to nutritious food, quality healthcare, or health education. The survey's numbers are a statistical echo of that unevenness.

A Policy Blind Spot

For decades, India's public health apparatus has been built primarily to fight undernutrition, through school meal programs, subsidized grain distribution, and maternal nutrition schemes. These efforts have had real impact, but the new data suggests they have not been enough to eliminate underweight prevalence, even as a parallel crisis of overweight and obesity has emerged largely unaddressed by the same institutions.

Public health systems geared toward one form of malnutrition are often ill-equipped to simultaneously manage its opposite. Obesity-related illnesses, including diabetes and cardiovascular disease, require different interventions, different messaging, and different clinical infrastructure than the treatment of underweight and its associated risks, such as anemia and stunted growth in children. India's health system, still catching up on the first front, now finds itself needing to fight a war on two fronts at once.

The survey's findings arrive as a warning to policymakers: India's health challenges cannot be addressed with a single, uniform strategy. As the country urbanizes further and its economy continues to grow unevenly, the coexistence of hunger and excess is likely to persist, and possibly deepen, unless public health policy evolves to address both realities at once.
