The American Academy of Pediatrics (AAP), the nation's leading professional group of children's doctors, is warning that a lesser-known front in the Trump administration's immigration crackdown is quietly endangering the health of children across the United States. Beyond the high-profile raids and deportations that have dominated headlines, the administration has pursued a strategy of restricting immigrant families' access to health care, food assistance and other public services, a shift pediatricians say is already causing measurable harm.

The AAP says the consequences fall heavily on children, including many who are American citizens but live in mixed-status households where a parent or sibling lacks legal residency. Doctors report that fear of encountering immigration enforcement, whether at a hospital, a school, or a government office, is now a significant deterrent keeping families away from care they need.

A Strategy of Deterrence Through Denial of Services

Unlike the visible tactics of workplace raids or expedited deportations, this approach works by making everyday institutions, clinics, welfare offices, schools, feel unsafe or inaccessible to immigrant families. Pediatricians say the effect is a chilling one: parents skip well-child visits, delay vaccinations, and avoid emergency rooms even when a child is seriously ill, out of fear that any interaction with a government-linked institution could trigger immigration scrutiny.

This is not the first time such fears have rippled through immigrant communities. Advocacy groups have long documented a "chilling effect" tied to policies like the expanded "public charge" rule from Trump's first term, which allowed immigration officials to weigh a family's use of public benefits, such as Medicaid or food stamps, against their applications for green cards. Even though that rule was narrowly written, its mere existence caused widespread confusion and led many eligible families to withdraw from benefit programs altogether, according to public health researchers who tracked its aftermath.

Why Pediatricians Are Especially Alarmed

Children are particularly vulnerable to gaps in care because early childhood is a critical window for vaccinations, developmental screenings and nutrition. The AAP has long emphasized that missed checkups and delayed immunizations do not just harm individual children; they raise the risk of disease outbreaks in entire communities, especially in areas where vaccination coverage begins to erode.

Pediatricians warn that fear-driven avoidance of health care does not stay contained within immigrant communities. Untreated infectious illness and lapsed immunization rates can spread risk to the broader public.

Mental health is another growing concern. Physicians report rising anxiety among children in immigrant households, some of whom worry constantly that a parent will be detained or deported while they are at school. Pediatric researchers have previously linked this kind of chronic stress, sometimes called "toxic stress," to long-term harm in child development, affecting everything from academic performance to immune function.

A Long History of Linking Immigration Policy to Public Health

The tension between immigration enforcement and public health is not new, but historians of health policy note that access to basic care has always been closely tied to a population's broader wellbeing and economic stability. In the mid-20th century, global health experts and organizations such as the United Nations recognized that consistent, affordable access to health care, including for vulnerable populations, was essential not just for individual welfare but for controlling the spread of disease and supporting long-term economic development.

Public health advocates argue that restricting care for immigrant children runs counter to those lessons. Failing to vaccinate or treat a segment of the population, they warn, can undermine collective immunity and place added strain on emergency health systems later, when preventable conditions go untreated and worsen.

What Comes Next

The AAP is calling for policymakers to separate immigration enforcement from the delivery of health care and public services, arguing that clinics, schools and hospitals should remain safe spaces regardless of a family's immigration status. The organization has previously issued guidance encouraging pediatricians to reassure immigrant families about confidentiality protections in medical settings and to advocate at the state and local level for policies that preserve access to care.

Whether the administration will adjust its approach remains unclear. For now, pediatricians say they are witnessing the consequences firsthand: children arriving at clinics sicker than they should be, missed vaccination schedules, and families increasingly reluctant to trust the institutions meant to protect their children's health.
